This elusive pressing has only recently been documented within the last year (2009-2010). Very few are known to exist.
A 1964 Indianapolis pressing with cover in shrink-wrap sold on eBay on July 4, 2012 for $107.38 plus shipping.3

This flat label pressing was discovered at auction on June 21, 2011 by someone who wishes to go by Anon. The winner provided scans of the labels, which have been replaced with this more recent find. Note that the matrix numbers are identical to the non-flat label. A second copy was found 6/15/2018. This is RARE.

Insert
8×10 photo of Elvis with the Elvis catalog on back. There are two variations of this insert. The first has a white border around the green background. The second photo7 has a green background stretched to the edge and is border-less. There are 11 RCA logos across the top of the image. The backs are identical.
